Reinventing Post-Acute Transitions of Care

Post-acute care transitions can be timely and complex. Care transition complexities are often associated with increased risk of hospital readmissions and poor resident outcomes. 
Recent regulatory changes, such as the Improving Medicare Post-Acute Care Transformation (IMPACT) Act, will place significant demand on facilities. Challenges include assuring medication-related problems at admission be handled in a timely manner. Experts will discuss the current state of post-acute care transitions, outline steps to improve the future of care transitions and provide solutions to meet today’s regulatory requirements.
